HCC – 09/26 – Provision of Support at Home Lead Provider Services

Hertfordshire County Council is currently out to procurement for the Provision of Support at Home Lead Provider Services. Support at Home delivers social care to those meeting Care Act eligibility criteria, based on a Connected Lives assessment carried out by the Council’s care management team. Services are delivered in an enabling manner and may be chargeable following a financial assessment. Support at Home is commissioned via a two-tier model, where the Lead Provider is referred all Support at Home packages in their district. Where a Lead Provider is unable to support, the remainder of the market is delivered by the Council’s Support at Home Spot Provider Framework. There will be one Support at Home Lead Provider for each lot area. A maximum of 4 Support at Home lots will be awarded to a single provider. under each contract type (ie a bidder could win 4 Reablement contracts and 4 Support at Home contracts maximum).
